What to Bring

Please bring the following with you to your appointment:

All of your glasses including sunglasses

Contact lens boxes

Insurance cards, medical, secondary, & vision coverage

If you take several medications, please bring an updated list

A copy of your completed Patient History Form (see Online Forms)

Payment as it is expected at the time of visit for eyewear purchases, contact lenses, and copays.

Please Note:

DILATING DROPS: If it is your first eye exam in our office, the doctor will probably put drops in your eyes to dilate your pupils. This allows the doctor to get a better view of the inside of the eye. The drops make you sensitive to light and last 2 -4 hours. If you are sensitive or will be coming from a distance, it is advisable to bring someone with you as a “driver”.  Examinations with the doctors are typically by appointment. You do not need an appointment to pick up glasses or contact lenses, but it is a good idea to call before you come so we can be better prepared to serve you in an expeditious manner.
